== Medical uses == Approved indication for dihydrocodeine is the management of moderate to moderately severe pain as well as coughing and shortness of breath. As is the case with other drugs in this group, the antitussive dose tends to be less than the analgesic dose, and dihydrocodeine is a powerful cough suppressant like all other members of the immediate codeine family (see below) and their cousins hydrocodone, oxycodone and ethylmorphine, whole opium preparations, and the strong opioid hydromorphone. For use against pain, dihydrocodeine is usually formulated as tablets or capsules containing 15–16 mg or 30–32 mg with or without other active ingredients such as aspirin, paracetamol (acetaminophen), ibuprofen, or others. Controlled release dihydrocodeine is available for both pain and coughing, as indicated below, as waxy tablets containing 60 to 120 mg of the drug. Some formulations, intended for use against coughing and the like, have other active ingredients such as antihistamines, decongestants and others. Other oral formulations, such as packets of effervescent powder, sublingual drops, elixirs and the like are also available in many locations. Injectable dihydrocodeine is most often given as a deep subcutaneous injection. Dihydrocodeine appears to be superior to tramadol in treating pain.
== History == Mycophenolic acid was discovered by Italian medical scientist Bartolomeo Gosio. Gosio collected a fungus from spoiled corn and named it Penicillium glaucum. (The species is now called P. brevicompactum.) In 1893 he found that the fungus had antibacterial activity. In 1896 he isolated crystals of the compound, which he successfully demonstrated as the active antibacterial compound against the anthrax bacterium. This was the first antibiotic that was isolated in pure and crystalline form. But the discovery was forgotten. It was rediscovered by two American scientists C.L. Alsberg and O.M. Black in 1912, and given the name mycophenolic acid. The compound was eventually demonstrated to have antiviral, antifungal, antibacterial, anticancer, and antipsoriasis activities. Although it is not commercialised as antibiotic due to its adverse effects, its modified compound (ester derivative) is an approved immunosuppressant drug in kidney, heart, and liver transplantations, and is marketed under the brands Cellcept (mycophenolate mofetil by Roche) and Myfortic (mycophenolate sodium by Novartis). Cellcept was developed by a South African geneticist Anthony Allison and his wife Elsie M. Eugui. == Clinical significance == Mutations in the COL3A1 gene cause vascular Ehlers-Danlos syndrome (vEDS; OMIM 130050), classified under this name in the 2017 International Classification of the Ehlers–Danlos Syndromes and formerly designated EDS type IV. It is the most severe form of EDS, since patients often die suddenly due to rupture of large arteries or other hollow organs. A few patients with arterial aneurysms without clear signs of EDS have also been found to have COL3A1 mutations. More recently, mutations in COL3A1 have also been identified in patients with severe brain anomalies suggesting that type III collagen is important for the normal development of the brain during embryogenesis. This phenotype resembles the cortical malformation caused by loss-of-function mutations in ADGRG1 (formerly GPR56; OMIM 604110), the gene encoding adhesion G protein-coupled receptor G1. Type III collagen is a known ligand for this receptor, linking the finding to a shared brain-development pathway. == Interactions == Methocarbamol may inhibit the effects of pyridostigmine bromide. Therefore, methocarbamol should be used with caution in those with myasthenia gravis taking anticholinesterase medications. Methocarbamol may disrupt certain screening tests as it can cause color interference in laboratory tests for 5-hydroxy-indoleacetic acid (5-HIAA) and in urinary testing for vanillylmandelic acid (VMA) using the Gitlow method.
== Side-effects == The most common side-effects are upper respiratory tract infection, diarrhea, combined edema/peripheral edema and headache, respectively. Most clinical adverse events were similar between groups treated with pioglitazone in combination with metformin and those treated with pioglitazone monotherapy.
